AGREEMENT FOR REGISTRATION OF A SCOLIOSISMANAGER ACCOUNT 1. ACCEPTANCE The service called "ScoliosisManager" is provided by ISICO Technology Services (hereinafter "ISICO") to each registered user (hereinafter "User"), in accordance with the terms and conditions contained in this Agreement. By completing the registration process and sending the request after selecting "Accept", you declare to have carefully read and accepted all the conditions and the terms of use of the Service as expressly set forth below. 2. DESCRIPTION OF SERVICE ISICO offers each user the ability to manage online data about their patients, keeping an electronic archive accessible via the Web, information on diagnosis, treatment, rehabilitation sessions, clinical evaluations. ISICO is in no way responsible for the use made by you of the tools provided in the service: it is solely your responsibility to verify the correct and appropriate use of these instruments in relation to the individual patient. No automatic tool (eg. The function "auto-composition" of work plans) can substitute the patient's assessment carried out personally by the operator to adapt therapeutic interventions to the needs and characteristics of the individual patient. 3. FREE ACCOUNT ISICO offers each user the ability to use free ScoliosisManager after registering an account. 4. PRE-TECHNICAL REQUIREMENTS The service, for use with different browsers, has been tested best with the Chrome browser. You must have, or acquire, however, at your own expense, the devices and programs needed to establish the connection with the Service. In particular, access to the service is essential to have an Internet connection with the appropriate characteristics: in this regard it is noted that the performance in terms of the program's response time to user input, will to some extent be proportional to wide transmission bandwidth provided by your ISP. 5. ACCOUNT SECURITY The User is solely responsible for the preservation and confidentiality of their service access credentials (login and password) and, consequently, remains solely responsible for all use of his/her account and / or password, whether they are from the same user authorized or unauthorized. In relation to the above, the User agrees to immediately notify ISICO any possible theft and / or loss and / or unauthorized use by third parties of their account and / or password as soon as being aware, however, committing as of now to indemnify and hold harmless from any and all ISICO request, also for damages, proposal and / or resulting, directly or indirectly, from the aforementioned use or misuse of the Service. ISICO never contacts the User to ask for username and password or other confidential information. Any request of this kind from third parties is considered as a lesion of confidentiality and must be reported to scoliosismanager@isico.it 6. SECURITY SYSTEMS ISICO is committed to ensuring the highest level of security in the user's data management by adopting computer safeguards such as firewalls and data encryption. The servers that host the program and the data are in a protected location where the data are entered into the program can only be accessed by authorized personal. All data inserted are subjected to daily backup mechanisms and periodic backups on remote servers. 7. LIMITATION OF LIABILITY You acknowledge and agree that ISICO can not in any way be held responsible for any damage, direct or indirect, from the User or to third parties: - The use, or inability to use the Service; - From any modification, suspension or discontinuance of the Service, or some features of it, for reasons attributable to ISICO or less; - Unauthorized access or alteration of transmissions and / or user data, including inter alia the possible damage or loss, by the User may incur for lost profits, use, data loss or other intangible elements; - The loss of the data entered for reasons attributable or not to ISICO and in particular following the automatic deletion of the data to the deadlines set for free accounts - The use of medical and scientific information contained in the program. They are expressly excluded all and any form of express or implied warranty to the extent that this does not contradict the current legislation and in no case ISICO can be held liable for failure to perform or inadequate performance by the User of any legal procedure or regulations in relation to that agreement. 8. INFORMATION ACCORDING TO THE CODE OF LAWS FOR PROTECTION OF PERSONAL DATA Pursuant to and by effect of article 13 of Legislative Decree no. 30 June 2003 n. 196 ("Code regarding the protection of personal data", hereinafter for brevity and the "Code"), the User declares to be informed fully and comprehensively about - The purposes and methods of processing of personal data freely communicated at the time of registration or in the course of execution of this Agreement, - The recording of such data on electronic media protected and treated confidentially by ISICO, - The compulsory or optional nature of providing data and the consequences in case of refusal to answer, - The scope of dissemination and categories of persons to whom such information may be disclosed only with the prior written consent, - The ownership of the treatments, - The rights set out in Articles 7 and following of the Code with particular regard to the possibility of access, integration, modification, deletion of personal data or opposition, in whole or in part to its use for the purposes specified therein, as specified below, by submitting formal request to ISICO, holder of personal data, at info@isico.it. With the signing of this Agreement, you give full consent to process personal data for all purposes imposed by legislative or regulatory obligations or injunctional and necessary and / or useful for the execution of this Agreement as well as for all activities however, related to the same agreement.Owner of data processing is ISICO Technologies Services - Corso Pavia 37-27029 Vigevano PV. This Agreement shall automatically be considered terminated if the information provided at registration resulted to be totally or partially false. Any amendments to the personal information provided at registration must be promptly notified to ISICO by e-mail through scoliosismanager@isico.it.
